Grade 4 prolapsed piles, 10 years of symptoms, treated with stapler haemorrhoidopexy (MIPH)
A man in his fifties with about ten years of a swelling coming out of the anal region and on-and-off bleeding. Examination showed a circumferential Grade 4 prolapse that could not be pushed back. A stapler lifted and fixed the prolapse with no external wound — chosen for this anatomy despite its higher prolapse recurrence than haemorrhoidectomy — and he was discharged the next day.

A trans-sphincteric fistula mapped on MRI and treated with hybrid fistulotomy + FiLaC
MRI fistulogram mapped a single trans-sphincteric tract from the 6 o’clock internal opening to a 4 o’clock external opening. The probe confirmed it in theatre, and the plan changed from a full lay-open to a hybrid fistulotomy + laser so the sphincter was not divided.
