Advanced & Recurrent Fistula Programme
Complex, recurrent and previously operated fistulas are handled as a dedicated programme. Almost every fistula is imaged before surgery — an MRI fistulogram or a trans-rectal ultrasound — because the tract's depth, branches and how much sphincter muscle it crosses cannot be judged from outside. The operation then follows the tract: fistulotomy for an early, simple, superficial tract; FiLaC laser, LIFT or VAAFT where the anatomy suits them; a combination technique with seton or laser when a deep tract involves more than half of the external sphincter; and staged treatment, seton first, for recurrent, multiple or horseshoe tracts. The same surgeon sees you from first consultation through surgery and every follow-up. Second opinions after failed surgery elsewhere are welcome.

Which laser procedures are used for piles, fissure and fistula?
For piles, laser haemorrhoidoplasty (LHP) is used in suitable grades. For chronic fissure, laser sphincterolysis relaxes the tight internal sphincter as an alternative to LIS. For fistula, FiLaC seals the tract with laser energy while preserving the sphincter muscle. Laser does not suit every case — grade 4 piles, for example, may need stapler surgery — so the choice follows examination.
