What Is a Perianal Abscess?

Perianal abscess anatomy diagram showing anal gland infection, pus cavity in perianal tissue, and potential fistula tract formation
Anatomy of a perianal abscess — infection of an anal gland leads to a pus-filled cavity in surrounding tissue.

A perianal abscess is a pus-filled infection that forms in the tissue around the anus or rectum. It usually starts when a tiny anal gland (located between the inner and outer sphincter muscles) gets blocked, infected, and fills with pus. Without treatment, the infection spreads, the abscess grows, and pressure causes intense pain.

Antibiotics alone do not cure most perianal abscesses — the pus must be surgically drained. The longer the abscess sits, the higher the chance it ruptures spontaneously and forms a chronic fistula-in-ano (a tunnel from the abscess to the skin).

Types of Perianal Abscess

  • Perianal abscess — Just under the skin near the anus. The most common type, usually drained with a small incision under local anaesthesia.
  • Ischiorectal abscess — Deeper in the buttock tissue. Needs drainage in operation theatre under spinal or general anaesthesia.
  • Intersphincteric abscess — Between the two sphincter muscle layers. Requires careful drainage to protect bowel control.
  • Supralevator abscess — High in the pelvis. Less common but more serious; needs imaging (MRI) and specialist surgical management.

Why do painful boils increase in summer?

Why do painful boils increase in summer? — Dr. Kundan Kharde, Sharvari Hospital Pune

Dr. Kundan Kharde explains why painful boils and abscesses near the anus flare up in summer — heat, sweating and friction let bacteria multiply in blocked glands — and why a painful, swollen boil that keeps growing needs drainage rather than waiting on antibiotics alone. Early treatment prevents complications. Results vary.

How We Treat Perianal Abscess

Step 1 · Within 1 hour

Examination & diagnosis

Dr. Kharde examines the affected area and confirms abscess location. For deeper or complex cases, an MRI or ultrasound is arranged immediately.

Step 2 · Same day

Incision & drainage (I&D)

Performed under local, spinal, or general anaesthesia based on abscess depth. A small incision releases the pus. The cavity is irrigated, dressed, and a soft drain may be placed for deeper abscesses.

Step 3 · Same-day discharge

Pain relief & antibiotics

Most patients go home the same day with pain medication, antibiotics if needed, and a clear wound-care plan. Sitz baths begin within 24 hours.

Step 4 · Follow-up (6-8 weeks)

Fistula monitoring

30-50% of patients develop a fistula tract after abscess drainage. We monitor for signs (recurrent discharge, persistent opening) and offer definitive laser fistula surgery if needed.

The Sharvari Difference

Your surgeon is your surgeon — even at 2 AM

An abscess can't wait — and you shouldn't be handed off to a rotating panel when it's most painful. At Sharvari Hospital, Dr. Kundan Kharde assesses and drains your abscess personally, then follows you through the 6–8 week fistula-surveillance window himself. The same surgeon, start to finish — not a call-centre coordinator and not an assigned junior.

One surgeon, start to finish

Dr. Kharde (MS, FMAS, 19 yrs) handles your same-day drainage and every follow-up himself — no rotating panel.

One clear quote, no surprise bill

A single written estimate before drainage — surgery, anaesthesia, day-care stay, and follow-up. No coordinator upsell added later.

A local hospital, not an aggregator

Sharvari Hospital is a single ISO 9001:2015 hospital in Pimple Nilakh with a 24/7 in-house OT — your treatment happens here.

Frequently Asked Questions

What is a perianal abscess?

A perianal abscess is a painful, pus-filled infection in the tissue around the anus or rectum. It usually develops when an anal gland gets blocked and infected. Without timely drainage, the abscess can grow, rupture spontaneously, and turn into a chronic fistula.

Is perianal abscess an emergency?

Yes — most perianal abscesses need urgent surgical drainage, ideally within 24-48 hours of symptoms. Delaying treatment increases pain, infection spread, and the risk of fistula formation. Sharvari Hospital provides 24/7 emergency surgical access.

How is perianal abscess treated?

The standard treatment is incision and drainage (I&D) under anaesthesia. The pus is evacuated, the cavity is irrigated, and the wound is left open or partially closed to drain. Antibiotics are added if infection has spread. Most patients go home the same day.

Will I develop a fistula after abscess drainage?

Around 30-50% of patients develop a fistula-in-ano after a perianal abscess. We follow up closely for 6-8 weeks. If a fistula tract develops, Dr. Kharde performs definitive sphincter-sparing laser or VAAFT surgery to close it.

How long is recovery after abscess drainage?

Most patients return to light activity within 3-5 days. Sitz baths, pain medication, and daily wound care guidance are provided. Full healing of the drained cavity typically takes 2-4 weeks.

What does perianal abscess drainage cost in Pune?

Incision and drainage costs start at ₹25,000-₹40,000 at Sharvari Hospital, depending on abscess size, anaesthesia type, and admission needs. Most major insurance policies cover it as an emergency procedure.

Recovery roadmap

Your Perianal Abscess Drainage (I&D) recovery — Day 1, 7 & 30

Perianal abscess drainage gives immediate pain relief. Healing happens by secondary intention over 2–4 weeks.

  1. 1

    Day 1 — Drainage Day

    Immediate pain relief

    • 20–30 min urgent incision & drainage under anaesthesia
    • Pus evacuated; cavity packed/left open to drain
    • Massive pain relief within hours
    • Same-day discharge with antibiotics
    • Sitz baths + wound packing taught to attendant
  2. 2

    Day 7 — First Follow-up

    Wound contracting

    • Daily sitz baths + wound irrigation
    • Wound size visibly smaller
    • Resume desk work by Day 3–4 for most patients
    • First in-person follow-up (free)
    • Monitor for fistula formation (10–30% risk)
  3. 3

    Day 30 — Healed

    Wound closed

    • Wound typically closed by secondary intention
    • Resume gym + normal activities
    • Final follow-up + fistula screening
    • If fistula tract develops → laser FiLaC planned electively
    • Long-term hygiene + recurrence prevention guidance

Note: Up to 30% of perianal abscesses develop into a fistula. Early follow-up at Sharvari Hospital catches this in time for elective laser repair.
